Congresswoman Rosa L. DeLauro’s Passport Day and Community Resource Fair!
Join us on Saturday, December 07, 2024 at Miller Cultural Complex located at 2901 Dixwell Ave for a day full of valuable resources.
The Community Resources Fair will take place from 10am to noon in Thornton Wilder Hall. This Event is free and open to the public.
The Passport Day Event is by APPOINTMENT ONLY, please call (203) 562-3718 to schedule your appointment. Appointments will be scheduled from 10am to 4pm and held in the Senior Center Social HAll.
What to Bring for Passports
Proof of U.S. Citizenship
Previous U.S. Passport (altered or damaged passports are not acceptable)
Certified Birth Certificate issued by the city, county or state of birth with official seal/certification, file number, date of filing and parent(s) full name
Consular report of birth abroad
Valid United States Naturalization papers; government issued, with photograph, and signed.
PROOF OF IDENTITY
One form of valid photo identification:
Driver’s license
Passport
Government or Military ID
Certificate of naturalization or citizenship
PASSPORT PHOTOGRAPHS
1 (2” x 2” in size) color photo, white or off-white background.
FEES
Passport Application Fee, payable by check or money order only. Make check/money order payable to: U.S. Dept Of State.
Credit or debit cards are also accepted.
There are additional fees for expedited passport service.
